To be considered for help from the Alexa Dyer Foundation the following criteria will be required:
-Child has been diagnosed with a terminal illness
-Child is 0 to 18 years of age when diagnosed terminal
-Child is US citizen or legally resides in Oregon and Southwest Washington
-Child has been diagnosed terminal by a Physician
-Application must be filled out by a Physician, Social Worker or Hospice Caregiver
-Family has financial need due to illness
The selection process guideliness are as follows:
The family in need of financial assistance due to the child's illness is referred or applies themselves. The application is submitted to the Alexa Dyer Foundation Committee for review. The Committee determines whether to approve the request for funding. If approved, the funds are distributed appropriately. If the request is not approved, the family will be contacted. A member of the committee will make follow up contact with the family.
